Coeliac disease is a lifelong autoimmune condition triggered by gluten in genetically susceptible people.
It can affect the small intestine, nutrient absorption, blood formation, bone health and other systems.
Pregnancy is possible for most women with coeliac disease.
The safest plan usually begins with four questions:
- Is the diagnosis secure?
- Is the woman following a genuinely gluten-free diet?
- Have nutritional deficiencies and associated conditions been identified?
- Is maternal weight, blood health and fetal growth being assessed appropriately?
The answer cannot be built from symptoms alone.
Some women have diarrhoea, bloating or weight loss.
Others present with:
- Iron-deficiency anaemia.
- Folate or vitamin B12 deficiency.
- Low bone density.
- Fatigue.
- Mouth ulcers.
- Abnormal liver tests.
- Neurological symptoms.
- Dermatitis herpetiformis.
- Subfertility.
- Recurrent pregnancy loss.
- A family history.
- Another autoimmune disease.
Some have few obvious symptoms despite active intestinal injury.
Others continue to have symptoms after gluten withdrawal because of:
- Accidental exposure.
- An incomplete original diagnosis.
- Lactose intolerance.
- Irritable bowel syndrome.
- Microscopic colitis.
- Pancreatic or bile-acid problems.
- Another gastrointestinal condition.
- Non-responsive or refractory coeliac disease.
- More than one diagnosis.
My approach begins with three principles:
- Confirm coeliac disease before removing gluten whenever diagnosis is still uncertain.
- Treat confirmed disease with a strict lifelong gluten-free diet—not occasional avoidance.
- Use pregnancy to assess nutrition, anaemia, associated autoimmunity and fetal growth without assuming that every complication is caused by coeliac disease.
No doctor can guarantee the outcome of a pregnancy.
Expert care can do something more useful:
- Reconstruct how the diagnosis was made.
- Identify whether the woman was eating gluten during testing.
- Review antibody and biopsy evidence.
- Distinguish coeliac disease from wheat allergy and non-coeliac gluten sensitivity.
- Find hidden gluten exposure.
- Assess iron, folate, vitamin B12, vitamin D, calcium and other relevant nutrients.
- Review weight and dietary adequacy.
- Identify thyroid, type 1 diabetes and other associated autoimmune conditions.
- Assess previous fertility and pregnancy outcomes.
- Plan maternal and fetal surveillance.
- Coordinate gastroenterology, dietetic and obstetric care when required.
- Prepare for postpartum nutrition and breastfeeding.
First Confirm the Diagnosis
A gluten-free diet is lifelong treatment for confirmed coeliac disease.
That commitment deserves a secure diagnosis.
Coeliac Disease Is an Autoimmune Enteropathy
In coeliac disease:
- Gluten exposure triggers an abnormal immune response.
- The lining of the small intestine may become inflamed and damaged.
- Intestinal villi may become shortened or flattened.
- Nutrient absorption may be impaired.
- Gastrointestinal and extraintestinal symptoms may occur.
Gluten is found in:
- Wheat.
- Barley.
- Rye.
- Foods and products contaminated by these grains.
Ordinary wheat, barley and rye are not made safe by:
- Sourdough fermentation.
- Organic farming.
- Traditional preparation.
- Small portion size.
- Eating them only occasionally.
- Taking digestive enzymes.
- Choosing a product labelled “low gluten.”
A person with confirmed coeliac disease requires a coeliac-safe gluten-free diet.
Coeliac Disease Is Not a Wheat Allergy
Wheat allergy is an allergic reaction to wheat proteins.
It may cause:
- Hives.
- Swelling.
- Breathing symptoms.
- Vomiting.
- Anaphylaxis.
- Exercise-associated reactions.
- Other allergic manifestations.
Coeliac disease is a different immune-mediated condition.
The diagnostic tests, emergency risks and dietary details are not identical.
A person may rarely have both.
The terms should not be used interchangeably.
Coeliac Disease Is Not Non-Coeliac Gluten Sensitivity
Non-coeliac gluten sensitivity describes symptoms associated with wheat or gluten-containing foods after coeliac disease and wheat allergy have been excluded.
It does not produce the same established coeliac autoimmune and intestinal-injury pattern.
Symptoms may also relate to:
- Fermentable carbohydrates.
- Other wheat components.
- Irritable bowel syndrome.
- Another gastrointestinal condition.
A response to gluten withdrawal does not prove coeliac disease.
Symptoms Alone Cannot Confirm or Exclude the Diagnosis
Possible symptoms include:
- Diarrhoea.
- Constipation.
- Abdominal pain.
- Bloating.
- Nausea.
- Vomiting.
- Reflux.
- Fatigue.
- Weight loss.
- Poor weight gain.
- Mouth ulcers.
- Headache.
- Neurological symptoms.
- Bone or muscle pain.
- Anaemia.
- Fertility or pregnancy concerns.
A woman may have coeliac disease without classic diarrhoea.
A woman may also have all these symptoms without coeliac disease.
Testing and clinical context matter.
Do Not Remove Gluten Before Testing When Diagnosis Is Uncertain
Current adult guidance recommends consuming a gluten-containing diet before diagnostic testing because serology and intestinal changes depend on exposure. [1,2,3]
Starting a gluten-free diet before assessment may:
- Lower antibody levels.
- Allow intestinal healing.
- Produce a false-negative result.
- Make biopsy interpretation difficult.
- Create uncertainty about whether lifelong restriction is necessary.
A person with suspected but unconfirmed disease should speak to the clinical team before removing gluten.
This does not mean that a pregnant woman should force gluten exposure without medical supervision when:
- She has a previous severe reaction.
- She has already been gluten free for a long period.
- Symptoms are substantial.
- Weight or nutrition is compromised.
- Another diagnosis may be present.
- A gluten challenge may not be appropriate during the current pregnancy.
The diagnostic strategy should be individualised.
First-Line Blood Testing
Adult diagnostic assessment commonly begins with:
- Immunoglobulin A tissue transglutaminase antibodies.
- Total immunoglobulin A.
This combination helps identify:
- A coeliac-associated antibody response.
- Selective IgA deficiency that could make an IgA-based test falsely reassuring.
Additional testing may include:
- Immunoglobulin A endomysial antibodies.
- Immunoglobulin G-based tests in IgA deficiency.
- Other tests selected by gastroenterology.
The laboratory method, upper limit of normal and degree of antibody elevation matter.
The word positive without the value and reference range is incomplete.
Selective IgA Deficiency Changes the Test Pathway
Coeliac disease is more common in people with selective IgA deficiency.
When total IgA is low, IgA-based coeliac serology may be falsely negative.
The clinician may use appropriate IgG-based testing and consider endoscopic assessment according to the complete picture. [1,2,3]
A negative IgA tissue-transglutaminase result should not be interpreted without knowing the total IgA level.
Antibody Levels Depend on Gluten Exposure
A negative result is less reliable when the woman has:
- Already stopped gluten.
- Reduced gluten substantially.
- Eaten gluten only intermittently.
- Used a restrictive diet for months or years.
- Had an incomplete gluten challenge.
- Taken immunosuppressive treatment that may alter the immune response.
- Selective IgA deficiency without appropriate IgG testing.
The laboratory result should be interpreted alongside actual diet at the time of testing.
“Gluten free” and “mostly gluten free” are not the same exposure history.
Duodenal Biopsy
Duodenal biopsies can assess:
- Increased intraepithelial lymphocytes.
- Crypt hyperplasia.
- Villous atrophy.
- The distribution and severity of intestinal change.
- Alternative diagnoses.
When biopsy is required, current BSG guidance recommends adequate sampling from the second part of the duodenum and the duodenal bulb because disease can be patchy. [1]
A normal-looking intestine during endoscopy does not exclude microscopic coeliac disease.
The tissue must be sampled and interpreted properly.
Selected Adults May Have a No-Biopsy Diagnostic Pathway
The 2026 BSG guideline allows an optional no-biopsy diagnosis in selected symptomatic adults in secondary care when immunoglobulin A tissue transglutaminase is at least ten times the assay’s upper limit of normal.
This pathway is conditional and should follow shared decision-making. [1]
It is not the same as:
- Diagnosing coeliac disease from mild antibody elevation.
- Diagnosing it from a home test.
- Diagnosing it after gluten has already been removed.
- Diagnosing it from symptoms alone.
- Allowing every adult to skip gastroenterology assessment.
Local guidelines and laboratory pathways differ.
The woman should understand exactly which diagnostic standard was used.
Pregnancy Can Change the Timing of Endoscopy
Pregnancy does not make coeliac disease unimportant.
It may change when non-urgent upper gastrointestinal endoscopy and biopsy are performed.
The decision should consider:
- Severity of symptoms.
- Anaemia.
- Weight loss.
- Gastrointestinal bleeding.
- Nutritional compromise.
- Concern for another serious diagnosis.
- Gestational age.
- Anaesthetic and procedural factors.
- Whether a validated serology-based pathway is available.
- Whether assessment can safely wait until postpartum.
A necessary investigation should not be withheld simply because the woman is pregnant.
An elective procedure should not be performed without a reason that justifies it.
HLA-DQ2 and HLA-DQ8 Can Help Exclude—Not Confirm
Most people with coeliac disease carry HLA-DQ2 or HLA-DQ8.
Many healthy people also carry one of these genetic patterns.
Therefore:
- A negative result makes coeliac disease very unlikely.
- A positive result does not prove coeliac disease.
HLA testing may be useful when:
- The woman started a gluten-free diet before testing.
- Serology and biopsy do not agree.
- The original diagnosis is uncertain.
- A specialist is planning whether a gluten challenge is necessary.
It should not replace first-line serology in an ordinary diagnostic pathway. [1,2,3]
Biopsy Reports Need More Than the Word “Inflammation”
A useful pathology report should describe:
- Number and site of specimens.
- Villous architecture.
- Crypt hyperplasia.
- Intraepithelial lymphocytes.
- Histological grade or equivalent description.
- Orientation and adequacy.
- Alternative or additional findings.
Villous atrophy can occur in conditions other than coeliac disease.
Increased intraepithelial lymphocytes without villous atrophy can also have several causes.
The diagnosis should combine:
- Diet at testing.
- Serology.
- Histology.
- Genetics when useful.
- Symptoms and deficiencies.
- Response to treatment.
- Exclusion of alternatives.
Potential Coeliac Disease Is Not the Same as Confirmed Enteropathy
Potential coeliac disease generally refers to persistent coeliac serology with:
- No villous atrophy.
- No major architectural change.
- A compatible HLA pattern.
Management may include:
- Specialist dietetic review.
- A trial of gluten-free treatment for symptoms.
- Continued gluten with planned reassessment.
- Repeat serology.
- Future biopsy if symptoms or results change.
The decision should not be made from one weakly positive test.
Pregnancy, nutritional status and symptom severity may affect timing, but they do not remove the need for diagnostic precision. [1]
Seronegative Coeliac Disease Requires Specialist Diagnosis
A small proportion of people may have coeliac disease despite negative standard antibodies.
The diagnosis requires:
- Villous atrophy.
- Appropriate genetic susceptibility.
- Exclusion of alternative causes.
- Clinical and histological response to a gluten-free diet.
- Specialist assessment.
A negative blood test with normal histology does not become “seronegative coeliac disease” simply because symptoms improve after removing wheat.
Dermatitis Herpetiformis Is a Coeliac-Spectrum Condition
Dermatitis herpetiformis is an intensely itchy blistering skin condition associated with gluten sensitivity.
Assessment may include:
- Dermatology review.
- Skin biopsy with direct immunofluorescence.
- Coeliac serology.
- Nutritional evaluation.
- Gastroenterology input.
A lifelong gluten-free diet is central treatment.
Medication may also be used under dermatology supervision.
An itchy pregnancy rash should not be labelled dermatitis herpetiformis without assessment because pregnancy has its own important skin and liver-related causes of itching.
A Past Diagnosis Should Be Reconstructed
I review:
- Why testing was performed.
- Whether the woman was eating gluten.
- Exact antibody values.
- Total IgA.
- Endomysial antibodies.
- Biopsy report.
- Number and site of biopsies.
- Histological description.
- HLA result when performed.
- Specialist conclusion.
- Date the gluten-free diet began.
- Symptom and laboratory response.
- Whether the diagnosis has ever been questioned.
A diagnosis from childhood may be correct.
It still helps to know how it was established because the pregnancy plan depends on confidence that lifelong restriction is necessary.
Home Tests and Commercial Sensitivity Panels Are Not Enough
I do not diagnose coeliac disease from:
- A home finger-prick test alone.
- Immunoglobulin G food-sensitivity panels.
- Hair analysis.
- Applied kinesiology.
- Stool “intolerance” panels.
- A wellness genetic report alone.
- Symptom improvement after a commercial elimination programme.
These approaches may:
- Mislabel ordinary foods.
- Create unnecessary restriction.
- Miss true coeliac disease.
- Delay assessment of another condition.
- Compromise pregnancy nutrition.
A valid diagnosis uses recognised clinical testing.
One Normal Test Does Not Answer Every Scenario
A negative test may be genuinely reassuring when:
- Gluten intake was adequate.
- The correct antibody test was used.
- Total IgA was assessed.
- Pre-test probability is not high.
- There is no major malabsorption or high-risk history.
Further specialist review may still be required when:
- Symptoms are strong or persistent.
- Iron deficiency is unexplained.
- Weight loss is substantial.
- A first-degree relative is affected.
- The woman has type 1 diabetes or another associated condition.
- Gluten intake was low.
- IgA deficiency is present.
- Serology and biopsy disagree.
The next step should be based on the whole diagnostic probability—not repeated random tests.
Before pregnancy care is built around lifelong gluten avoidance, establish whether the diagnosis is confirmed, probable, potential, untested or uncertain.
Pregnancy Should Begin With Coeliac Disease Treated
For confirmed coeliac disease, the treatment is a strict lifelong gluten-free diet.
Pregnancy is not the time to relax the diet.
It is also not the time to make the diet so restrictive that maternal nutrition becomes inadequate.
Why Treatment Before Conception Matters
Untreated or undiagnosed coeliac disease may be associated with:
- Nutrient deficiency.
- Anaemia.
- Low maternal weight.
- Bone disease.
- Menstrual disturbance.
- Subfertility.
- Pregnancy loss.
- Fetal growth concerns.
- Preterm birth.
- Low birth weight.
- Other adverse outcomes.
Much of the pregnancy evidence is observational.
It does not prove that coeliac disease causes every complication.
Risk appears more concerning when disease is untreated or unrecognised, and appropriate diagnosis with gluten-free treatment is an important part of care. [4,5]
Treated Disease Is Not the Same as Untreated Disease
A woman with:
- A secure diagnosis.
- A strict gluten-free diet.
- Corrected deficiencies.
- Appropriate weight.
- Stable symptoms.
- No major associated condition.
has a different pregnancy profile from a woman with:
- Ongoing gluten exposure.
- Unexplained anaemia.
- Weight loss.
- Active gastrointestinal symptoms.
- Persistent villous injury.
- Untreated thyroid disease.
- Type 1 diabetes.
- An uncertain diagnosis and multiple restrictions.
The label coeliac disease alone does not describe current control.
A Gluten-Free Diet Must Be Nutritionally Complete
A poor-quality gluten-free diet may be:
- Low in fibre.
- Low in iron.
- Low in folate.
- Low in vitamin B12.
- Low in calcium.
- Low in vitamin D.
- Low in whole-grain variety.
- High in sugar.
- High in saturated fat.
- High in salt.
- Dependent on ultra-processed replacement foods.
Removing gluten is essential.
Replacing it well is equally important during pregnancy.

- Avoiding bread but eating soy sauce containing wheat.
- Choosing “low gluten.”
- Taking enzymes before a meal.
- Having a small serving because symptoms are absent.
- Allowing regular cross-contact.
- Assuming pregnancy protects the intestine.
Symptoms do not measure intestinal injury reliably.
Some women feel no immediate reaction despite ongoing exposure.
A Dietitian With Coeliac Expertise Is Valuable
Specialist dietetic review may assess:

- Oats.

A deficiency should be identified and treated rather than covered by a general prenatal supplement alone.
Folate Requires Particular Attention
Pregnancy folate supplementation should follow national recommendations and individual risk.
A woman with coeliac disease may require additional assessment when there is:
- Folate deficiency.
- Persistent malabsorption.
- Severe dietary restriction.
- Previous pregnancy affected by a neural-tube defect.
- Anti-seizure medication.
- Diabetes.
- Higher body mass index.
- Another recognised indication.
A high dose should not be invented from a website.

Iron Deficiency Is Common—but Not Every Anaemia Is Iron Deficiency

- Investigation for another diagnosis.
No single test proves complete control in every adult. [1]
A Strict Diet Should Not Become an Unsafe Diet

- Removal of oats, pulses or grains without a reason.
- Very low carbohydrate intake.
- Vegan restriction without nutrient planning.
- Repeated detox programmes.
- Expensive unregulated supplements.

- Disordered eating.
- Inadequate pregnancy weight gain.
Coeliac treatment must be strict about gluten.
It should be flexible and nourishing everywhere else.
The preconception goal is not merely “gluten free.” It is confirmed diagnosis, reliable exclusion of gluten, restored nutrition and a pregnancy plan built around the woman’s actual risks.
How Coeliac Disease Can Affect Pregnancy
Coeliac disease can affect pregnancy through more than one pathway.
Possible pathways include:
- Intestinal inflammation.
- Nutrient malabsorption.
- Maternal underweight.
- Anaemia.
- Associated autoimmune disease.
- Placental mechanisms proposed in research.
- Delayed diagnosis.
- Dietary inadequacy after gluten withdrawal.
- Another coexisting condition.
The strength of evidence is not equal for every pathway.
Association does not prove that coeliac disease caused an individual pregnancy complication.
Many Women Have Uncomplicated Pregnancies
A diagnosis of coeliac disease does not mean that pregnancy will be complicated.

A negative test during little or no gluten exposure may be falsely reassuring.
A positive result obtained during ordinary gluten intake is interpreted differently from one obtained after months of restriction.

Please obtain the original report whenever possible.

That does not mean that every meal must be:

Exercise and Bone Health
When obstetrically safe, bone-supportive activity may include:
- Walking.
- Resistance exercise.

Anaemia should be treated for its own maternal and birth implications.
Autoimmune Liver Disease
Possible conditions include:

The pregnancy plan should review:

- Liver function.

Most Women With Treated Coeliac Disease Can Conceive
Population studies suggest that overall fertility in women with diagnosed coeliac disease may be similar to that of the general population.
Some studies have found:
- Little or no reduction in overall fertility.
- Reproductive disadvantage before diagnosis.
- More reassuring reproductive outcomes after diagnosis and treatment.
- Variation according to age and study design. [4,5]
A coeliac diagnosis should not be presented as infertility.
It should prompt:
- Diagnostic confidence.
- A strict gluten-free diet.
- Nutritional assessment.
- Review of associated autoimmune disease.
- A complete fertility evaluation when conception is delayed.
Untreated Disease May Affect Reproductive Health
Possible pathways include:

A useful pathway includes:
- Immunoglobulin A tissue-transglutaminase antibodies.
- Total immunoglobulin A.
- Appropriate additional testing when immunoglobulin A is deficient.
- Specialist confirmation before a lifelong gluten-free diet.
A fertility patient should not begin gluten avoidance immediately before testing because this may make the result unreliable.
Coeliac Testing Does Not Replace a Fertility Work-Up
A complete fertility assessment may include:
- Maternal age.
- Duration of trying to conceive.
- Ovulation.
- Ovarian reserve.
- Uterine anatomy.
- Tubal patency.
- Endometriosis.
- Polycystic ovary syndrome.
- Thyroid function.
- Prolactin when indicated.

A gluten-free diet should continue throughout fertility treatment when coeliac disease is confirmed.
The fertility clinic should know about:
- Gluten-free medication or supplement concerns.
- Anaemia.

- Food access during procedures or admission.
A Gluten-Free Diet Is Treatment—Not a Fertility Guarantee
Some women conceive after coeliac diagnosis and treatment.
That sequence does not prove that gluten was the only cause of infertility.

Current recurrent-miscarriage guidance also supports structured evaluation rather than attributing repeated loss to one unconfirmed explanation. [7,8]
Depending on the history, assessment may include:
- Detailed chronology of every loss.
- Ultrasound records.
- Fetal or pregnancy-tissue genetic results when available.
- Uterine assessment.
- Antiphospholipid-antibody testing.
- Thyroid assessment.
- Diabetes assessment.
- Genetic counselling or parental testing in selected cases.
- Review of later-loss or cervical factors.
- Coeliac testing while gluten is being eaten.
- Other tests guided by the clinical pattern.
Coeliac testing is one component.
It is not a replacement for recurrent-loss care.
Define the Pregnancy Losses Accurately
For each loss, document:
- Positive pregnancy test.
- Ultrasound confirmation.
- Gestational age.
- Fetal pole.
- Cardiac activity.
- Empty gestational sac.
- Biochemical pregnancy.

Antiphospholipid syndrome may cause:
- Recurrent pregnancy loss.
- Fetal death.
- Placental dysfunction.

Diagnosis requires recognised clinical and laboratory criteria.
Coeliac disease does not diagnose antiphospholipid syndrome.
A gluten-free diet does not treat it.
When antiphospholipid syndrome is confirmed, treatment follows its own specialist pathway.
Inherited Thrombophilia Testing Is Not a Coeliac Test
Routine thrombophilia panels should not be ordered simply because:
- Coeliac disease is autoimmune.
- A miscarriage occurred.
- A fetus was small.
- A family member had a clot without a clear history.
Testing should follow recognised indications.
An abnormal result should not lead automatically to anticoagulation.
Aspirin Is Not Automatic for Coeliac Disease
Low-dose aspirin may be recommended when a woman has recognised preeclampsia risk factors.
Coeliac disease alone is not a universal indication.

- Ultrasound findings.
- Local guideline.
- Clinical assessment.
A coeliac diagnosis alone is not the indication.
Observational Evidence and Miscarriage Risk
Meta-analyses and cohort studies have reported associations between coeliac disease and miscarriage, particularly before diagnosis or in untreated disease.

Treatment May Improve the Risk Profile
A strict gluten-free diet may improve:
- Intestinal inflammation.
- Nutrient absorption.
- Weight.
- Anaemia.
- Antibody levels.
- General health.
Observational data suggest that reproductive outcomes may be more reassuring after diagnosis and treatment than before diagnosis. [4,5]
This does not mean:
- Every previous loss was caused by gluten.
- A future loss is impossible.
- Antibody normalisation guarantees implantation.
- The diet replaces obstetric or fertility treatment.
Placental Mechanisms Are Still Being Studied
Research has proposed possible mechanisms involving:
- Maternal inflammation.
- Nutrient depletion.
- Anti-tissue-transglutaminase antibodies.

Fetal Growth Restriction, Small for Gestational Age and Low Birth Weight Are Different
Fetal growth restriction describes a fetus that may not be achieving its growth potential because of a pathological process.
Small for gestational age usually describes size below a population threshold.
Low birth weight describes birth weight below a defined absolute threshold.
A baby may be:
- Constitutionally small and healthy.
- Growth restricted.
- Low birth weight because of prematurity.
- Both preterm and growth restricted.
These terms should not be used interchangeably.
Coeliac Disease and Fetal Growth
Studies have reported associations between coeliac disease and:
- Fetal growth restriction.
- Small-for-gestational-age birth.

Associations appear more concerning in undiagnosed or untreated disease, though results vary between studies. [4,5]

A small fetus requires a complete growth assessment.

Coeliac disease should not be used to explain away hypertension or proteinuria.
Stillbirth Data Require Caution

Doppler is not a routine adherence test for coeliac disease.
It is an obstetric tool used when fetal or placental findings justify it.

3. Is another autoimmune or medical condition present?
If yes, follow that disease’s pregnancy pathway.
4. Is there current or previous placental disease?
If yes, plan growth, Doppler and fetal surveillance according to obstetric risk.
5. Will the proposed test change care?
If no, reconsider whether it adds value.
Surveillance should follow risk, not the label alone. Stable treated coeliac disease may need targeted nutritional follow-up within routine antenatal care. Untreated disease, low weight, anaemia, persistent symptoms, associated autoimmunity or placental history may justify closer maternal and fetal assessment.

Coeliac Disease Is Not an Automatic Indication for Induction
Induction may be recommended because of:
- Prolonged pregnancy.
- Hypertension or preeclampsia.

A coeliac diagnosis does not make vaginal birth unsafe by itself.

When the pregnancy is complicated by fetal growth restriction, birth timing should consider:

NICE advises against induction when fetal growth restriction is accompanied by confirmed fetal compromise and recommends offering caesarean birth instead. [12]

Stable treated coeliac disease alone does not automatically require continuous monitoring.
Intermittent Auscultation
When the woman and fetus remain low risk, intermittent fetal-heart-rate assessment may be appropriate according to local intrapartum guidance.
Escalation may be needed if:
- A risk factor develops.
- The fetal heart rate becomes abnormal.
- Meconium appears.
- Maternal condition changes.
- Labour complications occur.
The monitoring plan should be reassessed during labour.
Preterm Birth
When preterm birth is anticipated, management may include:

A separate contact allergy, latex allergy or dermatitis should be recorded independently.
Do not label every adhesive or topical product unsafe because it contains a wheat-derived ingredient without assessing actual exposure and allergy.
Blood Products
Coeliac disease does not prevent transfusion when clinically required.
Selective IgA deficiency may rarely have transfusion implications in people with relevant anti-IgA antibodies or previous reactions.
The team should review:
- Previous transfusion.
- Previous reaction.
- Known IgA deficiency.
- Immunology advice when relevant.
Necessary blood treatment should not be withheld because of dietary concerns.
Placenta and Cord Blood
Routine placental examination is not required solely because of treated coeliac disease.
Placental histology may be valuable with:
- Fetal growth restriction.
- Stillbirth.
- Severe preeclampsia.
- Abruption.
- Infection concern.
- Preterm birth.
- Another placental indication.
Routine cord-blood coeliac-antibody testing does not diagnose the newborn.
Delayed Cord Clamping
Delayed cord clamping should follow ordinary neonatal and obstetric guidance.
Maternal coeliac disease does not by itself alter the recommendation.

The 2024 ESPGHAN position paper found that breastfeeding pattern or duration has not been shown to reduce the child’s risk of developing coeliac disease, and feeding recommendations should not be altered for coeliac prevention. [13]
Breastfeeding remains valuable for many other maternal and infant reasons.
Maternal Gluten Intake During Lactation
For a mother with confirmed coeliac disease:
- Continue gluten exclusion.
- Maintain adequate energy.
- Maintain protein.
- Review calcium and vitamin D.
- Treat anaemia.
- Maintain hydration.
- Continue indicated supplements.
The 2024 ESPGHAN update found insufficient evidence to give special maternal-gluten recommendations during pregnancy or lactation for preventing coeliac disease in the child. [13]
This does not change the mother’s own need for a gluten-free diet.
Gluten Does Not Need to Be Removed From the Household Baby Diet at Birth
A newborn does not need:
- Gluten-free formula solely because the mother has coeliac disease.
- Coeliac blood testing at birth.
- A neonatal intestinal biopsy.
- Gluten-free medicines solely because of family history.
- A gluten-free maternal diet beyond the mother’s own medical treatment.
- A label of coeliac disease before exposure and testing.
The baby’s feeding should follow ordinary newborn guidance unless another diagnosis is present.
Breastfeeding Nutrition
Breastfeeding may increase maternal appetite and nutritional demand.
The plan should include:
- Regular meals.
- Gluten-free snacks.
- Protein.
- Calcium-containing foods.
- Vitamin D according to guidance.
- Iron treatment when required.
- Vitamin B12 in women at risk.
- Iodine review when dietary intake is limited.

A product marketed for milk supply is not automatically effective, safe or gluten free.

Commercial lactation products may contain:

- Barley malt.
- Ordinary oats.
- Shared-facility contamination.
- Herbal ingredients.
- High sugar.
- Unclear doses.
No food can guarantee milk supply.
Milk production depends on multiple factors including:
- Effective milk removal.
- Feeding frequency.
- Maternal and infant health.
- Breast anatomy and physiology.
- Medication.
- Birth factors.
- Support.
A gluten-free label should be checked.
A marketing claim should not replace lactation assessment.
Mental Health and Food Anxiety
Postpartum mental-health symptoms may include:
- Persistent sadness.
- Anxiety.
- Panic.
- Intrusive thoughts.
- Severe guilt.
- Inability to sleep when the baby sleeps.
- Loss of function.
- Thoughts of self-harm or harm to the baby.
- Psychotic symptoms.
Food anxiety may worsen because of:
- Hospital exposure.

- Multiple unnecessary restrictions.
Urgent mental-health assessment is required for severe or dangerous symptoms.
The woman should not be told that every emotional symptom is caused by gluten.
Family Support
Family members can help by:
- Maintaining a safe kitchen.
- Preparing gluten-free meals.
- Preventing crumb contamination.
- Reading labels.
- Providing safe hospital food when permitted.
- Supporting appointments.
- Avoiding pressure to eat unsafe foods.
- Protecting rest.
- Sharing infant care.
Support should reduce the mother’s workload.
It should not police or shame her diet.
The Baby’s Inherited Risk
A child with a parent who has confirmed coeliac disease has a higher risk than a child without an affected first-degree relative.
Family history does not mean that the child:
- Has coeliac disease at birth.
- Will definitely develop it.
- Must avoid gluten indefinitely.
- Requires immediate genetic testing.
- Requires routine neonatal serology.
The paediatric record should note the family history.
Future testing should be based on:
- Symptoms.
- Growth.
- Iron deficiency.
- Associated autoimmune disease.
- Age.
- Gluten intake.
- Local paediatric guidance.
HLA Testing in the Child
HLA-DQ2 and HLA-DQ8 testing may sometimes help identify a child in whom coeliac disease is very unlikely.
A positive result does not prove disease.
HLA testing is not required routinely for every newborn with one affected parent.
The decision may depend on:
- Family preference.
- Paediatric pathway.
- Multiple affected relatives.
- Type 1 diabetes research or screening programmes.
- Local resources.
Infant Feeding Should Follow Developmental Readiness
Complementary feeding should begin according to general infant-feeding guidance and developmental readiness.
The 2024 ESPGHAN update states that introducing gluten between completed 4 months and 12 months does not alter the cumulative risk of childhood coeliac disease. [13]
It does not recommend:
- Very early gluten exposure to prevent disease.
- Delaying gluten beyond 12 months to prevent disease.
- Introducing gluten while breastfeeding as a preventive strategy.
- A specific gluten-containing food type.
- A proven preventive gluten dose.
Parents should follow their paediatric and national feeding guidance.
Do Not Use a Home “Gluten Challenge” on the Baby
Introducing ordinary food during weaning is not the same as conducting a diagnostic gluten challenge.
A child with symptoms or abnormal testing should be assessed by paediatrics before gluten is removed.
Do not:
- Give large deliberate doses.
- Use symptoms alone to diagnose coeliac disease.
- Start a gluten-free diet before valid testing.
- Use commercial sensitivity panels.
- Diagnose the child because the mother is affected.
Breastfeeding Does Not Prevent or Cause Coeliac Disease
ESPGHAN reports that:
- Any breastfeeding versus none has not been shown to reduce childhood coeliac risk.
- Exclusive breastfeeding duration has not been shown to reduce risk.
- Breastfeeding during gluten introduction has not been shown to prevent coeliac disease. [13]
These findings should not be used to discourage breastfeeding.
They should remove false pressure from the mother.
Maternal Diet Does Not Need to Prevent the Child’s Coeliac Disease
The mother should follow her own medically required gluten-free diet.
A mother without coeliac disease does not need to remove gluten during pregnancy or breastfeeding solely to prevent coeliac disease in the baby.
A mother with coeliac disease should not reintroduce gluten for the baby’s benefit.
Current evidence does not support either preventive strategy. [13]
Formula Feeding
Formula choice should follow:
- Infant age.
- Growth.
- Allergy history.
- Medical need.
- Feeding tolerance.
- Paediatric guidance.
A standard infant formula is not changed solely because a parent has coeliac disease.
Wheat allergy, cow’s-milk allergy and coeliac disease are different conditions.
Symptoms in the Child That May Prompt Assessment
Depending on age, possible features include:
- Poor growth.
- Weight loss.
- Chronic diarrhoea.
- Constipation.
- Abdominal distension.
- Abdominal pain.
- Vomiting.
- Iron-deficiency anaemia.
- Fatigue.
- Mouth ulcers.
- Delayed puberty.
- Dental-enamel change.
- Dermatitis herpetiformis.
- Type 1 diabetes.
- Autoimmune thyroid disease.
- A strong family history.
These features are not specific.
Testing should occur while gluten is still being eaten unless a paediatric specialist advises otherwise.
Do Not Test the Newborn With Adult Serology
Newborn antibody results may reflect maternal antibodies and do not provide a valid diagnosis of childhood coeliac disease.
Routine neonatal testing is not recommended solely because the mother has coeliac disease.
The paediatric plan should focus on:
- Normal newborn care.
- Growth.
- Feeding.
- Family history.
- Future symptoms or risk-based assessment.
Vaccination
The baby should receive routine vaccinations according to national guidance unless a separate medical reason changes the schedule.
Maternal coeliac disease does not require:
- Delayed vaccines.
- A gluten-free vaccine schedule.
- Routine exemption.
- Alternative unregulated immunisation.
Maternal immunosuppressive medicines may affect selected live-vaccine decisions.
That question belongs to paediatrics and the prescribing specialist.
Postpartum Coeliac Follow-Up
Follow-up may include:
- Symptoms.
- Weight.
- Dietetic review.
- Coeliac serology when indicated.
- Full blood count.
- Ferritin.

Diagnosis Deferred During Pregnancy

- Another gastrointestinal condition is suspected.
- Mucosal healing needs assessment.
- Refractory disease is considered.
Breastfeeding does not automatically prevent endoscopy.
Medication, sedation and feeding advice should be discussed with gastroenterology, anaesthesia and paediatrics when needed.

Before another pregnancy, review:
