High Blood Pressure and Preeclampsia in Pregnancy

When a High BP Reading Is Not the Whole Diagnosis

A raised blood-pressure reading in pregnancy matters.

But one number does not tell us:

  • Why the blood pressure is high.
  • Whether the hypertension was already present before pregnancy.
  • Whether it began after 20 weeks.
  • Whether preeclampsia has developed.
  • Whether the kidneys, liver, brain, blood count or placenta are becoming involved.
  • Whether the baby is growing normally.
  • Whether treatment should begin.
  • Whether pregnancy can safely continue.

A woman with a blood pressure of 142/92 at 10 weeks is not in the same clinical situation as a woman who develops the same blood pressure for the first time at 32 weeks.

A woman with high BP but no protein in the urine may still require careful assessment.

A woman with protein in the urine does not automatically have preeclampsia.

And a woman with preeclampsia does not always have significant proteinuria.

The diagnosis comes from the timing, repeated blood-pressure pattern, baseline health, symptoms, urine, blood tests, placenta and baby—not from one reading alone.

My approach begins with three principles:

  1. Establish what the blood pressure was before the pregnancy changed it.
  2. Separate chronic hypertension, gestational hypertension and preeclampsia before deciding treatment.
  3. Protect the mother without losing sight of the placenta, fetal growth and the safest gestation for birth.

No doctor can guarantee that preeclampsia will never develop.

Specialist care can do something more useful:

  • Identify women at increased risk early.
  • Measure blood pressure correctly.
  • Establish a renal, liver, platelet and urine-protein baseline.
  • Distinguish chronic hypertension from new pregnancy-related disease.
  • Recognise superimposed preeclampsia.
  • Use aspirin prevention appropriately when indicated.
  • Treat hypertension without unnecessary delay.
  • Recognise severe disease before a crisis develops.
  • Monitor fetal growth and placental function.
  • Plan birth according to maternal and fetal risk rather than one number.
  • Continue surveillance after delivery, when hypertension and preeclampsia can still cause complications.
Establish the Diagnosis

First Establish Which Hypertensive Disorder This Is

“High BP in pregnancy” is not one diagnosis.

The principal possibilities include:

  • Chronic hypertension.
  • Gestational hypertension.
  • Preeclampsia.
  • Chronic hypertension with superimposed preeclampsia.
  • Severe hypertension requiring urgent treatment.
  • Hypertension secondary to another medical disorder.
  • White-coat hypertension in selected women.
  • Occasionally, more than one process occurring together.

The first question is therefore not simply:

“How high is the BP?”

It is:

“When did it become high, what was normal for this woman before that, and what else has changed?”

Chronic Hypertension

Hypertension is generally classified as chronic when it:

  • Was known before pregnancy.
  • Is present at the booking visit.
  • Is identified before 20 weeks.
  • Or the woman was already taking antihypertensive medication before pregnancy.

Chronic hypertension can be:

  • Primary hypertension.
  • Related to kidney disease.
  • Endocrine in origin.
  • Associated with vascular disease.
  • Related to another medical condition.

The diagnosis should not automatically be assumed to be pregnancy-induced simply because pregnancy was the first time somebody measured it carefully.

Gestational Hypertension

Gestational hypertension is new hypertension developing after 20 weeks without the diagnostic features required for preeclampsia.

It can remain gestational hypertension.

It can also progress to preeclampsia.

The diagnosis therefore describes the situation today.

It does not guarantee that the situation will remain unchanged tomorrow.

Blood Pressure Numbers

What Does BP 140/90 Mean in Pregnancy?

One of the most common searches is:

“Is BP 140/90 high in pregnancy?”

Yes.

Major obstetric guidelines generally define hypertension in pregnancy from a systolic blood pressure of 140 mmHg or higher and/or a diastolic pressure of 90 mmHg or higher.

But diagnosis usually requires confirmation with appropriately repeated measurements.

One Reading Is Not Always the Diagnosis

A single reading may be affected by:

  • Anxiety.
  • Recent walking.
  • Pain.
  • Talking during measurement.
  • An incorrect cuff size.
  • Unsupported arm position.
  • A full bladder.
  • Recent caffeine.
  • Poor equipment.
  • Incorrect technique.

A non-severe raised reading should therefore be interpreted after correct measurement and appropriate repetition.

Severe Hypertension Is Different

A blood pressure around 160 systolic or 110 diastolic or higher is a different clinical situation.

Severe hypertension during pregnancy or after delivery can increase the risk of:

  • Stroke.
  • Eclampsia.
  • Placental abruption.
  • Maternal organ injury.
  • Other serious complications.

It should not be left until the next routine appointment merely to see whether it settles.

What About BP 130/80 or 135/85?

Non-pregnant hypertension guidelines increasingly recognise lower BP thresholds.

Obstetric diagnostic definitions still generally use 140/90 mmHg for hypertension in pregnancy.

That does not mean a lower but rising blood pressure is clinically meaningless.

The interpretation may differ in a woman with:

  • Chronic kidney disease.
  • Previous severe preeclampsia.
  • Diabetes.
  • SLE.
  • APS.
  • A markedly lower baseline BP.
  • Other cardiovascular risk.

The number belongs in the whole clinical picture.

Timing Matters

High Blood Pressure Before and After 20 Weeks

The gestational age at which hypertension appears changes the diagnostic pathway.

High BP Before 20 Weeks

This usually raises the possibility of:

  • Pre-existing chronic hypertension.
  • Previously undiagnosed hypertension.
  • Kidney disease.
  • Secondary hypertension.
  • White-coat hypertension in selected women.

Preeclampsia classically develops after 20 weeks.

Very early preeclampsia-like presentations are unusual and require specialist assessment for an alternative or associated explanation.

High BP After 20 Weeks

New hypertension after 20 weeks raises the possibility of:

  • Gestational hypertension.
  • Preeclampsia.
  • Previously unrecognised chronic hypertension.
  • Another medical cause.

The next question becomes:

Is this blood pressure rise occurring alone—or is another part of the maternal or placental picture changing with it?

Clinical Reasoning

Gestational Hypertension Is Not the Same as Preeclampsia

Two women can both have a BP of 150/96 at 32 weeks.

They may not have the same diagnosis.

Woman A

BP 150/96.

No significant proteinuria.

Normal platelets.

Stable kidney and liver function.

No concerning symptoms.

Normal fetal growth.

Woman B

BP 150/96.

New significant proteinuria.

Falling platelet count.

Rising liver enzymes.

Severe persistent headache.

Woman C

BP 150/96.

No significant proteinuria.

Rising creatinine and pulmonary oedema.

Woman D

BP 150/96.

Normal maternal laboratory results.

Fetal growth restriction and abnormal placental Doppler.

The blood-pressure number is identical.

The pregnancy is not.

Gestational Hypertension Requires Follow-Up

A diagnosis of gestational hypertension should not be translated as:

“It is only BP.”

Women may subsequently develop:

  • Proteinuria.
  • Maternal organ dysfunction.
  • Severe hypertension.
  • Preeclampsia.
  • Fetal growth restriction.
  • Placental complications.

Follow-up matters because the diagnosis can evolve.

Proteinuria

Preeclampsia Can Occur Without Protein in the Urine

Another common search is:

“Can I have preeclampsia without protein in urine?”

Yes.

Significant proteinuria is one route to the diagnosis.

Preeclampsia may also be diagnosed when new hypertension after 20 weeks is accompanied by relevant maternal organ dysfunction even when significant proteinuria is absent.

Depending on the diagnostic framework, concerning features may include:

  • Kidney dysfunction.
  • Liver involvement.
  • Low or falling platelets.
  • Haemolysis.
  • Pulmonary oedema.
  • Important neurological symptoms.
  • Eclampsia.
  • Other evidence of significant systemic disease.

Some international classifications also incorporate uteroplacental dysfunction.

Why This Matters

A woman should not be falsely reassured simply because:

“There is no protein, so it cannot be preeclampsia.”

That statement is no longer sufficiently accurate.

The Opposite Is Also True

Protein in the urine does not automatically mean preeclampsia.

Proteinuria may occur with:

  • Chronic kidney disease.
  • Diabetic kidney disease.
  • Lupus nephritis.
  • Urinary infection.
  • Pre-existing glomerular disease.
  • Other renal conditions.
  • Occasionally transient circumstances.

The timing, BP and baseline renal findings matter.

Superimposed Disease

Chronic Hypertension and Superimposed Preeclampsia

This is one of the more difficult diagnostic situations.

A woman may begin pregnancy with:

  • BP 145/92.
  • Antihypertensive treatment.
  • Mild baseline proteinuria.
  • Chronic kidney disease.

At 30 weeks her BP may rise.

The question is not simply:

“Has her chronic hypertension become worse?”

The assessment should ask whether superimposed preeclampsia has developed.

I review:

  • Change from baseline BP.
  • New severe hypertension.
  • Need for escalation of antihypertensive treatment.
  • New or increasing proteinuria.
  • Platelet trend.
  • Creatinine trend.
  • Liver enzymes.
  • Neurological symptoms.
  • Pulmonary symptoms.
  • Fetal growth.
  • Amniotic fluid.
  • Placental Doppler where indicated.
  • Biomarker testing where available and clinically appropriate.

Baseline Proteinuria Changes the Interpretation

If protein was already present before 20 weeks, finding protein again at 30 weeks does not establish new preeclampsia.

The question becomes whether there has been:

  • A meaningful change in renal status.
  • New hypertension severity.
  • New organ dysfunction.
  • New placental dysfunction.
  • Another feature of preeclampsia.

This is why chronic hypertension and kidney disease should be documented properly early in pregnancy.

First Consultation

Reconstructing the Blood-Pressure Story

The first consultation should not begin with:

“Which tablet are you taking?”

It begins with the timeline.

I review:

  • BP before pregnancy when known.
  • BP at the first antenatal visit.
  • The gestational age at which BP first became abnormal.
  • Highest recorded systolic BP.
  • Highest recorded diastolic BP.
  • Whether readings were repeated.
  • Where they were measured.
  • Which device was used.
  • Cuff size.
  • Home BP records.
  • Clinic BP records.
  • Emergency or hospital readings.
  • Previous pregnancy hypertension.
  • Previous preeclampsia.
  • Previous HELLP syndrome.
  • Previous eclampsia.
  • Previous fetal growth restriction.
  • Previous placental abruption.
  • Previous indicated preterm delivery.
  • Previous stillbirth when relevant.

Review the Medical Background

I also review:

  • Chronic kidney disease.
  • Diabetes.
  • SLE.
  • Antiphospholipid syndrome.
  • Previous thrombosis.
  • Cardiovascular disease.
  • Endocrine disease.
  • Obesity.
  • Sleep apnoea when relevant.
  • Family history of hypertension.
  • Family history of preeclampsia.
  • Multiple pregnancy.
  • Assisted conception.
  • Current medicines.
  • Medicines taken immediately before pregnancy.

Antihypertensive Medication Before Pregnancy Matters

The medication list should establish whether the woman was taking:

  • Labetalol.
  • Nifedipine.
  • Methyldopa.
  • Another calcium-channel blocker.
  • A beta-blocker.
  • An ACE inhibitor.
  • An angiotensin-receptor blocker.
  • A diuretic.
  • Another antihypertensive.

Some medicines commonly used outside pregnancy require review when pregnancy is planned or recognised.

Do not stop essential BP treatment without arranging an appropriate replacement.

Measurement

A Blood Pressure Number Is Only as Reliable as the Measurement

One of the most useful things a woman can do at home is measure BP correctly.

A poor technique can create false reassurance or unnecessary alarm.

Before Measuring

When possible:

  • Sit quietly for about 5 minutes.
  • Empty the bladder.
  • Avoid measuring immediately after significant exertion.
  • Sit with the back supported.
  • Keep both feet supported rather than crossing the legs.
  • Support the arm at approximately heart level.
  • Do not talk during the measurement.

Use the Correct Cuff

A cuff that is too small can overestimate BP.

A cuff that is too large can also reduce accuracy.

An upper-arm monitor validated for pregnancy is preferable when available.

Record the Actual Numbers

Do not write only:

“BP normal.”

Record:

  • Date.
  • Time.
  • Systolic BP.
  • Diastolic BP.
  • Pulse when displayed.
  • Symptoms.
  • Medication timing if relevant.

Patterns are easier to recognise when the numbers are visible.

Home BP Does Not Replace Clinical Assessment

A reassuring home reading cannot assess:

  • Urine protein.
  • Platelet count.
  • Kidney function.
  • Liver involvement.
  • Pulmonary oedema.
  • Fetal growth.
  • Doppler.
  • Placental abruption.
  • Fetal wellbeing.

Home monitoring is a tool.

It is not the entire assessment.

Investigations

The Investigation Map

The aim is not to order the longest “preeclampsia profile.”

The aim is to answer specific questions:

  1. Is hypertension confirmed?
  2. Is preeclampsia present?
  3. Is an organ becoming involved?
  4. Is the condition worsening?
  5. Is the placenta affecting fetal growth or wellbeing?
  6. Is another diagnosis explaining the abnormal result?

1. Full Blood Count and Platelets

The platelet trend matters.

Falling platelets may suggest worsening disease, HELLP syndrome or another haematological problem.

The interpretation should consider:

  • Baseline platelet count.
  • Gestational thrombocytopenia.
  • Immune thrombocytopenia.
  • SLE.
  • APS.
  • Infection.
  • Liver disease.
  • Medication.
  • Haemolysis.

One low platelet count should not be labelled HELLP without the rest of the picture.

2. Kidney Function

Assessment may include:

  • Serum creatinine.
  • Electrolytes when indicated.
  • Previous creatinine results.
  • Urine protein.
  • Urinalysis.
  • Further renal assessment when clinically appropriate.

Pregnancy usually lowers serum creatinine.

A value that appears “within the non-pregnant laboratory range” may therefore still deserve attention in pregnancy.

3. Liver Assessment

Testing may include:

  • AST.
  • ALT.
  • Bilirubin.
  • Other tests according to the clinical situation.

Upper abdominal pain with abnormal liver tests requires careful assessment.

4. Haemolysis When HELLP Is Suspected

Depending on the clinical picture, assessment may include:

  • Peripheral smear.
  • LDH.
  • Bilirubin.
  • Other markers of haemolysis.

HELLP means:

Haemolysis, Elevated Liver enzymes and Low Platelets.

It should not be diagnosed from one liver enzyme result alone.

5. Urine Protein Quantification

When proteinuria needs quantification, a spot urine protein-to-creatinine ratio or albumin-to-creatinine ratio may be used according to local practice.

Twenty-four-hour urine collection is no longer routinely required in some major guidelines when reliable quantitative spot testing is available.

6. Fetal Assessment

Depending on gestational age and severity, assessment may include:

  • Fetal growth.
  • Abdominal circumference.
  • Estimated fetal weight.
  • Amniotic fluid.
  • Umbilical-artery Doppler.
  • Other Dopplers when clinically indicated.
  • CTG/non-stress testing.
  • Fetal movements.

7. Placental Assessment

The placenta should not disappear from the hypertension assessment.

I consider:

  • Fetal growth.
  • Amniotic fluid.
  • Umbilical-artery resistance.
  • Placental appearance when relevant.
  • Evidence of placental abruption.
  • Previous placental disease.

Preeclampsia is a maternal disease with important placental consequences.

Protein in Urine

What Does Protein in the Urine Mean During Pregnancy?

A woman often sees:

Protein: 1+

on a urine report and immediately searches:

“Does protein in urine mean preeclampsia?”

Not necessarily.

A Dipstick Is a Screening Test

Urine dipstick protein can be affected by:

  • Urine concentration.
  • Infection.
  • Blood in the sample.
  • Collection technique.
  • Other renal conditions.

When clinically relevant, protein should be quantified.

Common Quantitative Thresholds

Different laboratories and guidelines use different reporting units.

A commonly used threshold for significant proteinuria is approximately:

  • Protein:creatinine ratio 30 mg/mmol, or
  • approximately 0.3 mg/mg in another commonly used reporting system.

Some guidelines also use albumin:creatinine ratio.

The number should still be interpreted with the complete clinical assessment.

More Protein Does Not Automatically Mean More Severe Preeclampsia

Once preeclampsia has been diagnosed, the amount of urine protein alone should not determine the severity of the disease or the timing of birth.

Maternal symptoms, BP, organ function, fetal condition and gestational age matter more.

Protein Before 20 Weeks

Significant proteinuria early in pregnancy raises the possibility of pre-existing renal disease.

That baseline becomes extremely important if hypertension or additional proteinuria develops later.

Blood Tests

Which Blood Tests Are Done for Preeclampsia?

Another common search is:

“Which blood tests confirm preeclampsia?”

There is no single routine blood test that independently confirms every case.

The investigation looks for the maternal systems that preeclampsia can affect.

Platelets

A falling platelet count can indicate disease progression.

Creatinine

A rising creatinine may indicate renal involvement.

AST and ALT

Rising transaminases may indicate liver involvement.

Haemoglobin and Haemolysis Markers

These become particularly important when HELLP or another haemolytic process is suspected.

Trends Matter

A report should not be read in isolation.

For example:

Platelets 170,000 → 145,000 → 112,000

may tell a different clinical story from one stable platelet count of 145,000.

The same principle applies to:

  • Creatinine.
  • AST.
  • ALT.
  • BP.
  • Proteinuria.
  • Fetal growth.

Direction of change can matter as much as whether one value has crossed a laboratory reference line.

Biomarkers

What Are PIGF and the sFlt-1/PIGF Ratio?

Women increasingly search these terms after receiving a specialised preeclampsia blood test.

They are placental angiogenic biomarkers.

They can provide additional information in selected women with suspected preterm preeclampsia.

They Are Not a Replacement for Clinical Assessment

Different professional bodies use these tests somewhat differently.

NICE supports PIGF-based testing alongside standard clinical assessment in women with suspected preterm preeclampsia within specified gestational ranges.

ACOG has incorporated the sFlt-1/PIGF ratio as a complementary risk-assessment tool in selected settings, particularly for predicting progression to severe disease in appropriately selected hospitalised patients.

The test should not be interpreted as:

“Positive = definitely preeclampsia.”

or:

“Negative = nothing can happen.”

The Test Does Not Decide Delivery by Itself

Biomarker results belong alongside:

  • BP.
  • Symptoms.
  • Platelets.
  • Kidney function.
  • Liver tests.
  • Proteinuria.
  • Gestational age.
  • Fetal growth.
  • Doppler.
  • Fetal wellbeing.

A biomarker can refine risk.

It does not replace the pregnancy.

Prevention

Can Preeclampsia Be Prevented?

Not every case can be prevented.

Risk can sometimes be reduced.

Identify Risk Early

Important risk factors include:

  • Previous preeclampsia.
  • Chronic hypertension.
  • Chronic kidney disease.
  • Type 1 or type 2 diabetes.
  • SLE.
  • Antiphospholipid syndrome.
  • Multiple pregnancy.

Other factors used in risk assessment may include:

  • First pregnancy.
  • Maternal age.
  • Higher BMI.
  • Long interval since the previous pregnancy.
  • Family history of preeclampsia.
  • Other maternal and obstetric factors.

Professional guidelines differ slightly in how these are combined.

Low-Dose Aspirin

Low-dose aspirin has an established role in women at increased risk of preeclampsia.

It is usually started early in pregnancy—commonly around 12 weeks, with earlier initiation within the recommended first-trimester window favoured by some screening pathways.

The exact:

  • Dose.
  • Starting point.
  • Stop date.
  • Indication.

varies somewhat between guideline systems and individual clinical circumstances.

Aspirin should not be self-started merely because a woman is anxious about preeclampsia.

Why You May Hear Different Aspirin Doses

Different international recommendations use different low-dose aspirin regimens.

That does not mean one doctor has necessarily made an error.

The prescription should reflect:

  • The risk-assessment method used.
  • Maternal risk.
  • Gestational age.
  • Local guideline.
  • Bleeding considerations.
  • Other medication.

First-Trimester Preeclampsia Screening

Some centres use a combined screening model incorporating:

  • Maternal characteristics and history.
  • Mean arterial pressure.
  • Uterine-artery Doppler.
  • Placental growth factor.

This may help estimate the risk of preterm preeclampsia.

Other systems use clinical risk factors to decide aspirin prophylaxis.

A screening result predicts risk.

It does not diagnose future preeclampsia.

Calcium

Adequate calcium intake is important in pregnancy.

In populations or individuals with low dietary calcium intake, supplementation may reduce preeclampsia risk.

The need and dose should be based on the woman’s diet, medical background and local recommendations rather than taken indiscriminately.

What Does Not Replace Prevention

Preeclampsia cannot reliably be prevented by:

  • Drinking excessive water.
  • Bed rest.
  • Eliminating all dietary salt.
  • Avoiding normal activity without another indication.
  • Herbal remedies.
  • Home remedies marketed as BP cures.

Healthy nutrition and activity are valuable.

They are not substitutes for evidence-based prevention and surveillance.

Chronic Hypertension & Treatment

High BP Before Pregnancy Needs a Pregnancy-Specific Plan

Chronic hypertension should ideally be reviewed before conception.

The plan should address:

  • BP control.
  • Current medication.
  • Kidney function.
  • Baseline urine protein.
  • Diabetes.
  • Cardiovascular disease.
  • Secondary causes when relevant.
  • Aspirin eligibility.
  • Fetal-growth surveillance.
  • Risk of superimposed preeclampsia.

Treatment of Mild Chronic Hypertension Has Changed

Older practice often delayed treatment until BP became substantially higher.

Evidence from the CHAP trial changed this approach.

Major US professional guidance now supports treating mild chronic hypertension during pregnancy using 140/90 mmHg as the threshold for initiation or titration of treatment, rather than waiting for severe hypertension.

This did not increase the rate of babies being small for gestational age in that trial.

Medication Should Be Pregnancy Compatible

Commonly used medications may include:

  • Labetalol.
  • Nifedipine.
  • Methyldopa in selected circumstances.

The correct choice depends on:

  • Maternal heart rate.
  • Asthma.
  • Cardiac disease.
  • Kidney function.
  • Side effects.
  • Previous response.
  • Other medical conditions.

ACE inhibitors and angiotensin-receptor blockers generally require medication review when pregnancy is planned or recognised.

The correct response is not to stop BP medication and remain untreated.

It is to move to an appropriate pregnancy plan.

Treating High Blood Pressure Is Not the Same as Treating Preeclampsia

Antihypertensive medication lowers BP.

It does not remove the underlying placental disease responsible for preeclampsia.

This distinction matters.

Why Treat the BP?

The immediate maternal objective is particularly important when BP reaches severe levels.

Treatment reduces the risk of catastrophic complications such as stroke.

Why Can Preeclampsia Still Progress After BP Improves?

A tablet may bring BP from 165/112 to 138/86.

That is valuable.

It does not prove that:

  • Platelets have recovered.
  • Liver disease has resolved.
  • Kidney involvement has resolved.
  • Pulmonary oedema cannot develop.
  • Placental function is normal.
  • The baby is growing normally.

BP control and disease surveillance must therefore happen together.

Do Not Chase Every Single Home Reading With Extra Tablets

Antihypertensive medication should not be repeatedly self-adjusted in response to isolated home readings unless a specific clinician-led plan has been provided.

Over-treatment can also cause problems.

Patterns and clinical context matter.

Safety

Which Preeclampsia Symptoms Need Urgent Assessment?

Preeclampsia can be asymptomatic.

That is why routine BP measurement matters.

When symptoms do occur, some deserve urgent assessment.

These include:

  • Severe or persistent headache.
  • New visual disturbance.
  • Blurred vision.
  • Flashing lights or spots.
  • Severe upper abdominal or right-sided pain.
  • Persistent epigastric pain.
  • Severe nausea or vomiting with other concerning features.
  • Sudden breathlessness.
  • Chest symptoms.
  • Markedly reduced urine output.
  • New neurological symptoms.
  • Seizure.
  • Feeling acutely or unusually unwell.
  • Significant sudden swelling accompanied by other concerning features.
  • Reduced fetal movements.

Does Swelling Mean Preeclampsia?

Not by itself.

Swelling of the feet and ankles is common during pregnancy.

More concerning is:

  • Rapidly increasing swelling.
  • Facial or hand swelling.
  • Swelling occurring with high BP.
  • Headache.
  • Visual symptoms.
  • Upper abdominal pain.
  • Breathlessness.
  • Other evidence of preeclampsia.

The diagnosis should not be made from ankle swelling alone.

Does Headache Mean Preeclampsia?

Not every headache in pregnancy is caused by preeclampsia.

Concern increases when the headache is:

  • New.
  • Severe.
  • Persistent.
  • Associated with elevated BP.
  • Associated with visual symptoms.
  • Associated with neurological symptoms.
  • Not behaving like the woman’s usual headache pattern.

The symptom should be interpreted, not automatically labelled.

Severe BP Requires Urgent Attention Even Without Symptoms

Feeling well does not make severe hypertension safe.

What Can High BP and Preeclampsia Mean for the Baby?

The effect on the baby depends substantially on placental function and gestational age.

Possible complications include:

  • Fetal growth restriction.
  • Reduced placental blood flow.
  • Abnormal Doppler findings.
  • Reduced amniotic fluid in some pregnancies.
  • Placental abruption.
  • Preterm birth.
  • Need for neonatal care.

A Normal-Weight Baby Does Not Make Severe Maternal Disease Safe

Fetal growth may remain normal even when maternal preeclampsia is becoming severe.

The mother’s condition matters independently.

A Small Baby Does Not Automatically Prove Preeclampsia

Fetal growth restriction has several possible causes.

When a small baby is accompanied by maternal hypertension, the possibility of placental disease and preeclampsia becomes particularly important.

The Placenta Links the Two Assessments

Maternal and fetal surveillance should not be run as separate files.

The clinical picture may include:

rising BP → maternal laboratory change → placental dysfunction → reduced fetal growth → abnormal Doppler

or it may not.

The pattern determines the plan.

Surveillance

Maternal and Fetal Surveillance

The intensity of monitoring depends on:

  • Diagnosis.
  • Gestational age.
  • BP severity.
  • Symptoms.
  • Laboratory results.
  • Medication requirement.
  • Fetal growth.
  • Doppler.
  • Amniotic fluid.
  • Fetal wellbeing.
  • Distance from hospital and ability to return promptly.
  • Previous disease pattern.

Maternal Monitoring May Include

  • Repeated BP.
  • Symptom review.
  • Urine protein when relevant.
  • Platelets.
  • Creatinine.
  • AST and ALT.
  • Other blood tests according to the clinical picture.

Fetal Monitoring May Include

  • Maternal awareness of fetal movement.
  • Ultrasound growth assessment.
  • Amniotic-fluid assessment.
  • Umbilical-artery Doppler.
  • Additional Doppler according to the fetal condition.
  • CTG/non-stress testing when indicated.

Monitoring Is Not Treatment by Itself

A reassuring scan today does not guarantee that preeclampsia cannot worsen tomorrow.

Likewise, one abnormal result does not automatically mean immediate delivery.

Surveillance exists to identify direction of change and to recognise the point at which continuing pregnancy becomes less safe.

Timing of Birth

When Should the Baby Be Born?

This is often the most important question.

It cannot be answered from the BP alone.

The decision balances:

the benefit of gaining gestation

against

the risk of continuing a pregnancy affected by worsening maternal or placental disease.

Around 37 Weeks

For women with preeclampsia who do not already have a reason for earlier birth, 37 weeks is an important threshold in major guidelines.

At this stage, continuing pregnancy usually offers less benefit relative to the increasing maternal and placental risk.

Before 37 Weeks

Delivery may be required earlier if there is:

  • Uncontrolled severe hypertension.
  • Eclampsia.
  • Progressive maternal organ dysfunction.
  • HELLP syndrome.
  • Pulmonary oedema.
  • Significant neurological deterioration.
  • Placental abruption.
  • Severe fetal compromise.
  • Other maternal or fetal indications.

Preeclampsia at 32 Weeks Does Not Automatically Mean Immediate Delivery

This is another common search.

At an early gestation, selected women who are clinically stable may sometimes be managed expectantly in an appropriate specialist setting.

The assessment must consider:

  • Presence or absence of severe features.
  • Maternal stability.
  • Laboratory trends.
  • Fetal growth.
  • Doppler.
  • CTG.
  • Availability of maternal and neonatal intensive care.
  • Ability to respond immediately if the condition deteriorates.

The purpose of waiting is not simply:

“Keep the baby inside for as long as possible.”

The purpose is:

Gain useful gestation only while continued pregnancy remains safer than birth.

Steroids When Preterm Birth Is Likely

When preterm birth is anticipated within the relevant gestational window, antenatal corticosteroids may be recommended to improve neonatal outcomes.

Treatment for fetal maturity should not delay a delivery that is urgently required for maternal or fetal safety.

Labour and Birth

Does Preeclampsia Mean Caesarean Delivery?

No.

Preeclampsia alone does not automatically require Caesarean birth.

The mode of birth depends on:

  • Gestational age.
  • Cervical status.
  • Previous births.
  • Previous Caesarean.
  • Fetal presentation.
  • Fetal wellbeing.
  • Urgency of delivery.
  • Maternal condition.
  • Obstetric indications.

Induction of labour may be appropriate for many women.

Caesarean birth may be safer in other circumstances.

Blood Pressure Still Matters During Labour

Maternal BP should continue to be monitored and treated appropriately.

Preeclampsia does not stop because labour has begun.

Why Is Magnesium Sulfate Given in Preeclampsia?

Magnesium sulfate is not primarily a blood-pressure medicine.

Its major obstetric role in preeclampsia is seizure prevention in women for whom the risk of eclampsia warrants treatment.

It is also used to treat eclamptic seizures.

Does Every Woman With Preeclampsia Need Magnesium?

No.

The indication depends on:

  • Severity.
  • Severe features.
  • Symptoms.
  • Gestational stage.
  • Clinical setting.
  • Whether eclampsia has occurred.
  • Professional guideline and individual assessment.

Magnesium Does Not Treat the Entire Disease

It reduces seizure risk.

The woman still requires:

  • BP management.
  • Maternal surveillance.
  • Laboratory monitoring.
  • Fetal surveillance.
  • Delivery planning.

HELLP Syndrome and Eclampsia Need Their Own Attention

Eclampsia

Eclampsia is the occurrence of a seizure related to the hypertensive disorder of pregnancy when another neurological explanation does not better account for it.

This is an obstetric emergency.

HELLP Syndrome

HELLP describes:

  • Haemolysis.
  • Elevated liver enzymes.
  • Low platelets.

Women may present with:

  • Upper abdominal pain.
  • Nausea.
  • Vomiting.
  • Headache.
  • Feeling profoundly unwell.
  • Laboratory abnormalities.

Not every woman with HELLP has the classic combination of dramatic hypertension and heavy proteinuria before the syndrome becomes apparent.

Atypical presentations are one reason symptoms and laboratory changes should not be ignored simply because one BP reading appears reassuring.

What Can I Do at Home to Lower BP in Pregnancy?

This is one of the most commonly searched questions.

The answer requires care.

Useful Actions

Depending on the pregnancy, useful measures may include:

  • Take prescribed medication consistently.
  • Measure BP correctly when home monitoring has been advised.
  • Keep a written BP record.
  • Attend scheduled maternal blood tests.
  • Attend fetal-growth and surveillance appointments.
  • Maintain appropriate hydration.
  • Eat a nutritionally balanced diet.
  • Maintain normal safe mobility unless another condition requires restriction.
  • Avoid smoking and nicotine.
  • Seek assessment for warning symptoms.

What Should Not Replace Medical Care

Do not rely on:

  • Lemon water.
  • Garlic.
  • Beetroot alone.
  • Coconut water.
  • Herbal preparations.
  • Severe salt restriction.
  • Bed rest.
  • Excessive water intake.
  • Unprescribed supplements.

to treat significant pregnancy hypertension or preeclampsia.

A food may be part of a healthy diet.

It is not an emergency antihypertensive treatment.

Bed Rest Is Not a Cure

Routine prolonged bed rest is not an established treatment for preventing or curing preeclampsia and may create other risks, including reduced mobility and thrombosis.

Activity should be individualised according to maternal and obstetric circumstances.

After Birth

Preeclampsia Can Begin After Delivery

Delivery does not mean the maternal risk disappears immediately.

Preeclampsia can:

  • Persist after birth.
  • Worsen after birth.
  • Occasionally appear for the first time after delivery.

Postpartum preeclampsia is therefore an important diagnosis.

Symptoms After Delivery Still Matter

Seek urgent assessment for symptoms such as:

  • Severe headache.
  • Visual disturbance.
  • Severe upper abdominal pain.
  • Breathlessness.
  • Chest symptoms.
  • Seizure.
  • Markedly elevated BP.
  • Feeling seriously unwell.

Tell the clinician that you have recently delivered a baby, even if pregnancy itself is over.

BP Medication May Continue

Some women require antihypertensive medication for days or weeks after delivery.

Treatment should be adjusted according to postpartum BP and breastfeeding considerations.

Proteinuria and Abnormal Blood Tests Need Follow-Up

Persistent:

  • Proteinuria.
  • Kidney dysfunction.
  • Liver abnormalities.
  • Hypertension.

should not simply be attributed indefinitely to the pregnancy.

Follow-up may reveal previously unrecognised chronic hypertension or kidney disease.

Future Health & Next Pregnancy

The Pregnancy Ends. The Cardiovascular Risk Does Not Completely End With It.

A history of preeclampsia is associated with a higher later-life risk of:

  • Chronic hypertension.
  • Cardiovascular disease.
  • Stroke.
  • Other vascular disease.

This does not mean that future cardiovascular disease is inevitable.

It means that the pregnancy has revealed information about long-term vascular health.

After Recovery

Long-term care should include appropriate attention to:

  • Blood pressure.
  • Weight.
  • Physical activity.
  • Smoking.
  • Diabetes risk.
  • Lipids when clinically indicated.
  • Kidney health.
  • Primary-care follow-up.

A complicated pregnancy should become part of the woman’s permanent medical history.

The Next Pregnancy Begins With the Previous One

After preeclampsia, I review:

  • Gestational age at diagnosis.
  • Maximum BP.
  • Whether severe hypertension occurred.
  • Proteinuria.
  • Platelet nadir.
  • Creatinine.
  • Liver involvement.
  • HELLP.
  • Eclampsia.
  • Fetal growth.
  • Doppler.
  • Placental abruption.
  • Gestational age at delivery.
  • Placental histopathology when available.
  • Postpartum BP.
  • How long medication was required.
  • Whether hypertension eventually resolved.

Recurrence Risk Is Individual

Recurrence risk is higher when previous disease was:

  • Early.
  • Severe.
  • Associated with preterm delivery.
  • Associated with chronic hypertension.
  • Associated with kidney disease.
  • Associated with diabetes or autoimmune disease.

A previous preeclampsia diagnosis should lead to planning.

It should not lead to the prediction:

“It will definitely happen again.”

Before the Next Pregnancy

The plan may include:

  • BP assessment.
  • Kidney function.
  • Baseline urine protein.
  • Medication review.
  • Weight and metabolic health.
  • Review of previous records.
  • Aspirin-prevention strategy.
  • Early antenatal booking.
  • First-trimester risk assessment when available.
  • A surveillance plan.

Hope Must Be Based on the Pattern

Many women with chronic hypertension, gestational hypertension or preeclampsia have good maternal and neonatal outcomes.

The prognosis depends on:

  • Gestational age.
  • Type of hypertension.
  • Severity.
  • Speed of progression.
  • Maternal organ involvement.
  • Response to BP treatment.
  • Fetal growth.
  • Placental function.
  • Availability of specialist maternal and neonatal care.
  • Timing of birth.

Preeclampsia Does Not Always Mean Immediate Premature Delivery

Some pregnancies can safely continue under close surveillance.

Others cannot.

Controlled BP Does Not Mean Preeclampsia Has Disappeared

Medication can control the BP while the underlying disease continues to evolve.

One Abnormal Test Does Not Automatically Mean the Pregnancy Must End

The result must be interpreted with:

  • Maternal stability.
  • Gestational age.
  • Laboratory trend.
  • Fetal condition.
  • The complete clinical picture.

The Safest Gestation Is Not Always the Longest Gestation

Every additional day inside the uterus has potential value for a premature baby.

Every additional day also carries potential risk when maternal or placental disease is worsening.

The aim is therefore not simply to continue pregnancy.

The aim is to continue pregnancy while continued pregnancy remains the safer option.

High BP Is a Number. Preeclampsia Is a Clinical Pattern.

A blood-pressure reading tells us something important.

It does not tell us everything.

BP 142/92 does not tell us:

  • Whether hypertension existed before pregnancy.
  • Whether it is gestational hypertension.
  • Whether preeclampsia has developed.
  • Whether proteinuria was already present.
  • Whether platelets are falling.
  • Whether the kidneys or liver are becoming involved.
  • Whether the placenta is functioning normally.
  • Whether the baby is growing normally.
  • Whether birth is safer today or whether useful gestation can still be gained.

The safest plan comes from connecting those pieces.

The objective is not simply:

“Bring the BP down.”

It is to:

  • Define the hypertensive disorder correctly.
  • Prevent severe maternal complications.
  • Detect progression early.
  • Protect placental and fetal wellbeing.
  • Use medication when it improves safety.
  • Gain gestation when gaining gestation remains safe.
  • Deliver when waiting has become the greater risk.
  • Continue maternal surveillance after birth.
  • Use the pregnancy to improve the woman’s future cardiovascular health.

One number starts the assessment.

The pattern determines the plan.

Bring your BP record, previous pregnancy details, urine reports, blood tests and ultrasound reports. Leave with a maternal, fetal and delivery surveillance plan.
